In order to try-out for Cedarbrook sports teams, students must submit a completed PIAA sports physical and sign up using the season specific interest form. A new interest form will be posted before each sports season. A sports physical is not the same as the regular physical/well check. Sports physicals for the 2025-2026 school year are ONLY valid if completed after April 30, 2025. Sports physicals may be scanned and emailed to Mr. Van Ooyen and Mr. Nestor or dropped off at Cedarbrook’s main office. Section 6 of the sports physical must be completed by the student’s doctor. Sections 1 - 5 must be completed by the student and their guardian. Students may not attend try-outs or participate on a team without this important document.  Thank you!
